Responsibilities:

· As a Licensed Mental Health Therapist, you will play a crucial role in the well-being of our clients.

· Conducting psychosocial assessments of new patients and updating detailed reassessments when necessary.

· Maintaining contact with the patient’s family and significant others when appropriate, providing education, support, and advocacy.

· Coordinating and acting as a liaison with community-based social and mental health agencies.

· Facilitating discharge planning and conducting post-discharge follow-up to ensure the patient’s successful transition to community resources, reducing the risk of re-hospitalization.

· Leading daily therapy groups for patients.

· Documenting all client interactions and maintaining confidentiality as per company and regulatory guidelines.

Qualifications:

· Education: Bachelor’s degree or higher in Social Work or a related field is required.

· Preferred licensure: LCSW, LMSW, LPC, LMHC, LICSW or LMFT.
